AHCCCS programs include (continued from Pg 1):
SOBRA for Children - This program is available for pregnant women and children up to the age of 19 who have a maximum income level ranging from 100% to 140% of the Federal Poverty Level (FPL), depending on total family size. Covered services include doctor's office visits, hospitalization, prescriptions, lab work, and behavioral health services.
Medicare Cost Sharing - This program is available to individuals who are age 65 or older, blind, or disabled and is provided as a supplement to Medicare expenses. Various plans are available, subject to income level, and cover payment of the Medicare Part A and Part B premiums, deductibles, and coinsurance. Special requirements include eligibility for Medicare Part A hospital insurance, but there is no limit on personal assets such as stocks and investments.
SSI Cash/Medical Assistance - This program is available to individuals who are age 65 and over, blind or disabled, but who are not eligible to receive monthly cash benefits under the Supplemental Security Income program. The plan provides medical coverage such as doctor's office visits, hospitalization, prescriptions, lab work, and behavioral health services. Income restrictions apply to this plan, but there are no limits on personal assets such as stocks and investments.
Arizona Long Term Care - This program is available to individuals who are age 65 and over, blind, or disabled, and provides a nursing facility level of care on an ongoing basis, including services provided on an in-home basis or in an assisted living environment. Both income and total asset restrictions apply to this program.
Healthcare Group of Arizona - This program is available to small businesses, ranging from sole proprietors to small businesses with less than 50 employees. Three comprehensive HMO plans are available with monthly premiums starting at around $100. Qualification criteria include: 1) at least 60 days of business has been conducted within Arizona, 2) the sole proprietor or small company must be presently uninsured, 3) insured members must work a minimum 20 hours per week, and 4) a minimum percentage of an organization's employees must be enrolled in the plan.
